IMPORTANT DETAILS:
This form is solely a registration form and not a formal contract. How-
ever, herein stated terms and/or information are made part of said formal
contract with FWOW Magazine Online.

Once you have filled out FWOW Water Cafe' e-TV! Registration
Form---you must contact our offices, preferably at that time, to discuss
your contract with FaithWalksOnWater Magazine Online and the legal    
details regarding use of your video(s) online. As time restraints do not
necessarily permit---we may or may not contact you, upon receipt of your
registration form.

Our contract is concise---which is to state in short---we're simply "compensating
you, for use of your video online, at said rate". However note that in making
contract with FaithWalksOnWater Magazine Online---we retain the sole right,
thus licensure, to use your video(s) for the length of time FaithWalksOnWater
Magazine Online and for the length of time our parent magazine, Unlimited
Blacks In America Magazine remains online, and/or uses said video(s) for
herein stated purposes; thus note that our parent magazine may make use of
said video(s) submitted by Principal Actor'(s) and/or images/pictues/ names/
and/olr voices---soley for herein stated marketing, advertising, and/or
promotional purposes.

However---take note, said use(s) by our parent magazine result in your advan-
tage, given any additional exposure---thus increased visibility may present us
with opportunity to increase your compensated rate(s).

Also note that your contract with FaithWalksOnWater Magazine Online       
also includes---using your video(s), said video(s)  name(s), all image(s) and     
picture(s) derived from said video, any names of person(s)  named as "Principal
Actors", and any or all voice(s)---in any and/or all marketing, promotional, and
advertising material(s) created for FaithWalksOnWater Magazine Online and   
our parent magazine Unlimited Blacks In America Magazine Online.

Further note that only those persons named as "Principal Actors" will   
be compensated. Any verbal or written agreement(s) you have with other
individual(s)---who appear in your video(s)---are in no contractual basis, way,
form, or form of use of said person(s) image(s) appearance(s), or voice(s) in said
video(s)---the responsibility of FaithWalksOnWater Magazine Online or the
responsibility of our parent magazine, Unlimited Blacks In America Magazine
Online.

Thus please take note: Any use of any minor(s) in said video(s), become the
sole responsibility of said Principal Actor(s), however FWOW Magazine must
have on record a signed Affidavit of  where, when, and how you generate your
scenes; and when necessary---exact a "Release Of Use Form and Errors
Omission Form", as we example here.

FaithWalksOnWater Magazine Online reserves the right to refuse any said   
video(s) submitted, before, during, or after said contract has been made.

FaithWalksOnWater Magazine Online reserves the right to to continue or
discontinue use of said video(s) submitted, at anytime.
